This program is an evidence-based model that provides assertive and intensive care to engage individuals with serious and persistent mental illness into personalized, recovery-focused treatment, support and rehabilitation. The team includes a nurse, prescriber, therapists, recovery support specialists and more. Care is provided in the community through a highly individualized approach that provides adults with the tools they need to obtain and maintain safe housing, employment, relationships and relief from symptoms and medication side effects.
This program enables people with an acquired brain injury to live outside of institutions by providing community-based, non-medical services. Individuals must be eligible for services through either the Connecticut Department of Social Services, ABI Medicaid Waiver or the Department of Mental Health and Addiction Services ABI.
This service provides settings for adults with serious mental illness to interact socially in a recovery-orientated environment. Vocational assessment, job placement and coaching, peer support and social and recreational activities are offered to empower individuals to develop skills that lead to gainful employment in the larger community. Individuals are referred to this program through other programs within the CHR system.
These mental health and substance abuse rehabilitation services offer the support necessary to assist people in achieving and maintaining the highest degree of independent functioning.
We offer court-ordered treatment for those with domestic violence offenses.

This program identifies and transitions those who would otherwise be cared for in a nursing facility to more appropriate community-based services.
This program assists individuals in exploring and realizing their educational and employment goals. Services include comprehensive vocational assessments, individual and group counseling and coordination with the business community to broaden each individual’s opportunities. CHR is launching efforts to enhance services for veterans, members of the armed forces and family members who are experiencing mental health and substance use challenges. This effort, known as CT First, is part of a new and far-reaching federally funded initiative to expand mental health, substance use and integrated wellness services in the region. These case management and outreach services for young adults (18-25) support their integration into the community. Assistance includes help with establishing a living situation, clinical services and education and/or vocational planning. Individuals referred for services come through Department of Mental Health and Addiction Services and are usually youth who have been in residential treatment through the Department of Children and Families, in foster care and/or in the juvenile justice system.
